Michael Houlihan co-founded Evergreen Rehabilitation in 2000. He currently serves as President and CEO. Previously, Mr. Houlihan has served in various leadership roles in long-term care & rehabilitation including Chief Operating Officer, Regional Vice President, and Nursing Home Administrator. Mr. Houlihan is a graduate of the University of Kentucky, and he currently resides in Louisville, KY with his wife Stacy and their two children.

Douglas Hall, Executive Vice President, co-founded Evergreen Rehabilitation in 2000. Mr. Hall’s career in healthcare began in 1992 as a Nursing Home Administrator and he eventually moved into leadership roles, including Regional Vice President of Operations and Vice President of Administration at a publicly traded rehab provider. Mr. Hall is a graduate of the University of Kentucky, and he resides in Louisville, KY with his wife, Carrie Pearson Hall, and their four children. In his leisure time, Mr. Hall is a competitive Triathlete and multiple Ironman® Triathlon finisher.

Rex Paggeot, Chief Financial Officer, began his career in long-term care in 1974 as a Nursing Home Administrator. This role ultimately evolved into corporate leadership and ownership for skilled nursing with Health Maintenance Organizations Inc., and assisted living, home health care, and rehabilitation services with Homestead Health Management Group Inc. He joined Evergreen Rehab as CFO via merger in 2005. Mr. Paggeot is a graduate of the University of Tampa and is married to Bonnie Paggeot. He has three children.

Barbara J. Reed, Chief Operating Officer, began with Evergreen Rehabilitation in 2002 as a Physical Therapist and has held various leadership roles in the Company including Vice President of Operations, Regional Director, and Rehab Director. She is a native of Mainz, Germany and originally came to the United States in 1993 to experience a “one year” adventure working abroad. That adventure continues! Barbara is a Registered Nurse and a Physical Therapist, and she currently lives in Chattanooga, TN.

Liz Barlow, Vice President of Clinical Services, joined Evergreen in 2001. Liz is a Registered Nurse and Certified Nurse Executive, and she holds other certifications in Rehab Nursing and MDS. Her professional background includes long-term care management and rehabilitation management. She lives in Kentucky with her husband, Todd, and their two sons. Leisure activities include cooking, tennis, and reading.

Donna Campbell, Vice President of Recruiting, has been with Evergreen Rehab since December 2006. Having been active in healthcare recruiting for more than 20 years, she has held senior and director positions at several large healthcare organizations, acquiring a broad management background. Donna holds a Bachelor of Science degree from the University of Louisville and a Professional in Human Resources Certification from the Society for Human Resources Management. She enjoys spending time with her two sons and extended family.

Shawn Halcsik, Director of Compliance, holds a Doctorate in Physical Therapy from Marymount University, Masters Degree in Education with an emphasis in adult and continuing education from Carroll College, and Bachelor of Science in Physical Therapy from Saint Louis University. Prior to joining the Evergreen Rehab Team in 2010, Shawn was a Senior Medical Reviewer and Claims Analyst at TrustSolutions, LLC, a Medicare Program Safeguard Contractor (PSC), providing subject matter expertise in coding, reimbursement, documentation, Medicare regulations, and practice guidelines to internal and external customers including FBI, OIG, AG, and AUSA across multiple states.

Leslie Dearing, Controller – Director of Human Resources, has been with Evergreen since May of 2005. Leslie graduated from the University of Louisville and has held accountancy positions with L&N Federal Credit Union and Atria Senior Living. Leslie and her husband, Bradley, welcomed their first child in February 2011.
